MANAGEMENT OF YOUR HEALTH INFORMATION
Regular patients of this practice are able to obtain visits in their home, residential aged care facility, residential care facility or hospital, both within and outside our normal opening hours where such visits are deemed safe and reasonable and where the patient is: acutely ill, immobile or elderly, has no means of transport to the practice, and if disabled access to the practice is unsuitable. Please enquire with reception regarding this service.
This centre operates on an appointment basis, walk-ins are accepted for urgent appointments, however there may be an unavoidable wait as priority is given to patients who have made booked appointments with their Doctors. There is no guarantee which Doctor will be allocated to walk-in patients. All patients should present to reception to be triaged by the Practice Nurse. At times you may experience a waiting period due to Doctors tending to urgent or complex medical issues. We apologise for any inconvenience.

Forms can be downloaded from this website or are located on the Reception desk at the centre. Should you wish to make a complaint, please contact our Operations Manager in the first instance. Alternatively, complaints can be made to The Office of Health Review (www.healthreview.wa.gov.au).
